職域接種における保冷ボックス「J-BOX BIO MISSION」の活用について

About the use of the cold storage box "J-BOX BIO MISSION" for occupational vaccination

On the 8th of this month, the government started accepting applications from companies and universities that inoculate the new coronavirus vaccine. As of 5 pm on the same day, it seems that there have been applications for 414 venues. On the other hand, I think that there are quite a few companies who are struggling with preparations due to the lack of information from the government.

Here, we will summarize the guidelines of the Ministry of Health, Labor and Welfare on workplace vaccinations and try to guess the image of the site.

First, let's take a look at the " Guidelines for Occupational Vaccinations (First Edition) " issued by the Ministry of Health, Labor and Welfare.

First of all, in the mass media, there are many places that use the term “workplace vaccination” instead of “occupational vaccination”. In this blog, I will consider it as a workplace inoculation.

Next, let's take a look at the vaccines used in workplace vaccinations this time.

The manual contained the following statement.

“The COVID-19 vaccine used in this occupational vaccination is a Takeda / Moderna vaccine , and its characteristics are shown in the table below . The minimum delivery unit for the company's vaccine is 100 doses, so it is necessary to maximize the number of people who can be vaccinated per site per day so that the number of doses per vial can be effectively used. Therefore , in principle, it is desirable to be able to secure a system for administering more than 100 doses of vaccination on the day of vaccination . Scheduling vaccinations for 105 people is less likely to generate fractions (assuming that 10 doses can be vaccinated with one vial, there will be fractions of 5 doses x 3 for the former and 5 doses x 1 for the latter).

It is the Takeda/Moderna vaccine that was approved last month, not the Pfizer vaccine used for individual vaccination. Please check the characteristics of each company's vaccine in the table below.

Storage temperatures and storage periods for Takeda/Moderna vaccines are as follows.

・-20 °C±5 °C: 6 months

・2 to 8°C: 30 days

On the other hand, the packaging of Takeda/Moderna vaccines delivered to companies and universities is as follows.

・Vial size: diameter 24mm x height 50mm

・Vial box size: width 51mm x depth 126mm x height 60mm

Each vial box contains 10 vials ( for 100 uses) .

In addition, for medical institutions that carry out vaccination in large volumes, 12 vial boxes (size is the same as above) ( 1200 doses) are delivered in a packaging box (size: width 169 mm x depth 267 mm x height 135 mm) . increase.

I think it's easier to understand if you look at the diagram below. Basically, each company or university receives a red-framed outer box (= vial box). For high-volume deliveries, the Shipping Case (packing 12 outer boxes) on the right side will be used for delivery.

There are other precautions when handling Takeda/model vaccines, so let's take a look! !

■Temperature control of Takeda/Moderna vaccine

・Temperature when delivered to each company or university: -20°C±5°C (can be stored for 6 months)

★ Storage before opening is the same

・Decompression method

① 2 and a half hours in an environment of 2 to 8 ° C (return to room temperature before using)

(2) 1 hour in an environment of 15 to 25°C

・Preservation method after thawing (before puncture)

(1) 2 to 8°C environment (up to 30 days, return to room temperature before use)

(2) Environment at 8 to 25°C (up to 12 hours, use within valid period)

・Preservation method after thawing (after puncture)

①Environment at 2 to 25°C (discard after 6 hours or more after first use)

However, there are things that need attention other than temperature control.

Please check the figure below.

Yes, it is "shading".

- Minimize exposure from room lighting

・Avoid direct exposure to sunlight and ultraviolet rays.

Regarding the above, regarding the outer boxes and vials of Takeda / Moderna vaccines,

It is unknown how much light blocking performance it has.

Were you able to understand the characteristics of the Takeda/Moderna vaccine?

So, let's imagine the image of the actual workplace vaccination.

First, information from the Ministry of Health, Labor and Welfare.

https://www.mhlw.go.jp/content/000789163.pdf

There are three patterns of workplace vaccination.

Pattern 1: Conducted at company clinics

Pattern 2: Implemented by visiting an external organization

Pattern 3: Implementation by visiting an external organization

There are 3 patterns. Larger companies have in-house clinics,

Most SMEs do not have in-house clinics, right?

In that case, I think that workplace vaccination centered on patterns 2 and 3 will become mainstream.

Let's take a look at the image of setting up the venue.

Information is summarized in an easy-to-understand manner in the setup image diagram of the venue.

However, it does not mention "vaccine storage location". On the other hand, according to the "Guidelines for Occupational Vaccinations Regarding the Implementation of Vaccinations for New Coronavirus Infections (First Edition) ", an ultra-low temperature freezer at -20 degrees is recommended for storing Takeda/Moderna vaccines.

To summarize,

- Requires a dedicated freezer

・A dedicated outlet is required to use the freezer.

・A maximum of 2400 doses (240 vials) can be stored in the dedicated freezer.

・It is necessary to secure current capacity and starting power.

Since it is necessary to secure a dedicated outlet, current capacity, starting power, etc., in Japan where there are many natural disasters, I think that it will be a considerable burden on each company and university.

In addition, the guidance of the Ministry of Health, Labor and Welfare also included the following description.

“In particular, the minimum delivery unit for Takeda / Moderna vaccines is 100 doses, so we are trying to maximize the number of people who can be vaccinated per site per day so that we can effectively utilize the number of doses per vial. It is necessary to increase the number of vaccinations, and it is desirable to be able to secure a system for administering 100 or more doses of vaccination on the day of vaccination.”

As I wrote before, after unpacking,

(1) 2 to 8°C environment (up to 30 days, return to room temperature before use)

Therefore, I do not feel the need for an ultra-low temperature freezer.

・When using an ultra-low temperature freezer, it is necessary to secure a dedicated outlet, current, and power.

・There is a risk of storage accidents due to power outages

・Transportation of vaccines from storage locations to vaccination venues within companies and universities

・Light shielding, vibration

・Increase the number of people who can be vaccinated per site per day as much as possible.

Considering the above problems, I think that the burden on the site will be less if we use our "J-BOX BIO MISSION" cooler box for vaccines.

When using our 2-8°C refrigeration box:

・When using an ultra-low temperature freezer, it is necessary to secure a dedicated outlet, current, and power.

→Because we use ice packs that can be frozen even in general refrigerators, refrigerators in the company can be used as they are, and there is no need to secure dedicated outlets, current, and power.

・There is a risk of storage accidents due to power outages

→No storage accidents due to power outages due to the use of ice packs

・Transportation of vaccines from storage locations to vaccination venues within companies and universities

→ Depending on the number of people vaccinated, it is possible to move only the cooler box, the inner bag or the vial holder.

・Light shielding, vibration

→By using a cushioned vial holder and an inner bag that can store the outer vial box, you don't have to worry about light shielding or vibration.

Please check the following.

30 vials can be stored. In J-BOX BIO MISSION, by default,

With 2 vial holders, it can hold up to 60 vials.

Inner bag that can store the outer box of Takeda / Moderna vaccine as it is,

Please check the following.

In J-BOX BIO MISSION, by default,

With 2 inner bags, it can store up to 8 outer boxes (80 vials).

Moreover, it comes with a handle, making it ideal for intra-company transportation.

*Additional cushioning material is required.
